feat: offline-first run command + .gitignore (closes #4)#9

#4) stackbilt run is now fully offline by default — no API key or network required. buildScaffold() replaces the auto-gateway path. - Default: local buildScaffold() (zero network, <1ms) - --gateway: explicit opt-in to Stackbilt gateway (TarotScript tier-2) - --persist: POST scaffold to platform account (requires API key) - --oracle: request LLM polish on persisted scaffold (requires --persist) Bumps @stackbilt/scaffold-core dep to ^1.2.0.
